Overview

What is a Good Faith Estimate?

A Good Faith Estimate provides an estimate of the expected cost of scheduled services. Because therapy is individualized, the total cost of care may vary depending on the frequency and duration of services.

Katelynn Leisher, LCSW-C provides services on a self-pay basis and does not currently bill insurance.

Requesting

How to request an estimate

You may request a Good Faith Estimate when scheduling services or when otherwise applicable under federal law.

The estimate will reflect the services expected at the time it is prepared. If your treatment needs or schedule change, your actual charges may differ.

Your rights

If your bill is higher than your estimate

If you receive a bill that is at least $400 more than your Good Faith Estimate, federal law may provide a process for disputing the charge.

For information about your rights and the federal patient-provider dispute process, visit the Centers for Medicare & Medicaid Services.
